Connect your Employee Portal or Virtual Assistant to your Formstack account with the Formstack App.

With the Formstack App, you can display any form created in your Formstack interface in your Virtual Assistant (Chatbot), so that every employee in your organization can access them easily. The Virtual Assistant will automatically read the form’s fields and render them conversationally, in a simple discussion.

This app works both for standalone Formstack Forms, or forms that are connected to Formstack Documents.

If a user starts filling in a form but doesn’t have time to finish it, no worries, all data will be saved (stop & saved feature) so they can resume it later.

This app lets you build various conversational experiences, either by giving access to a specific form or set of forms to your users in response to a question, or as a complete welcome experience that directs them to fill the form they need right at the start of their interaction with the chatbot.

Prerequisites for installing the Formstack App

To install this app, you must first get a Formstack Access Token. Find more information on generating an access token in Formstack on this page: https://help.formstack.com/s/article/generate-an-access-token-in-Formstack-Forms.

You will need to add this access token in the corresponding field in the app's configuration fields.

Folder Mode

In Folder mode, the Formstack app will display the list of all the available forms in a specific folder. To add or remove forms, you need to add them in a folder in your Formstack backoffice. Click here to learn more about folders on Formstack.

Installation:

  1. Create a new article by clicking on Knowledge Base > New Article, and give it a title (i.e "What are my services?").

  2. Add the app by clicking on Create a new block > Business App > Formstack

  3. Select "folder" in the Mode field to activate this mode

  4. Enter the name of the folder where you saved your forms in the Folder Name field. By default, it will be set to "Clevy", but you can change this value to any folder of your choice, or leave it empty for the root directory!

  5. If your folder contains forms that are linked to Formstack Documents, you can also enter "documents" in the Folder Type field

Single Form Mode

Using Single Form Mode you can alternatively bind this app to a specific form in response to an Article.

Installation:

  1. Create a new article by clicking on Knowledge Base > New Article, and give it a title (i.e "I would like to start working remotely")

  2. To add the App, click on Create a new block > Business App > Formstack

  3. Enter "single" in the Mode field

  4. Add the Formstack form id in the Form ID field

To find the Form ID, go to your Formstack Backoffice, open the Settings page for the form you wish to display, then copy the number showing in the page address between settings and general (without the two slashes). In the below example, where the address is https://www.formstack.com/admin/form/settings/4853924/general, the form ID is 4853924.

Welcome Mode

This app also comes as a fully ready-to-use Welcome Experience app, which guides your user to perform one of the most common actions: filling a form, generating a document, or asking a question.

Installation

  1. In your Virtual Assistant channel settings page, edit the Welcome Message

  2. Create a new Business App block and select the Formstack Forms Welcome Experience App in the list

  3. Enter the folder names you are intending to use for simple forms and documents (leave the field empty if you don't want to use one of them)
